Medical Social Worker
Do you or someone you know suffer from trauma or phobias due to an illness or injury? Do not let negative emotions come between you and the world! Forever Young’s Medical Social Workers primary goal is to work with our patients to help them through their toughest times. Our medical social workers visit patients in their homes in Philadelphia, Bucks County and Montgomery County.
Sometimes the trauma associated with these types of tragedies limit us from enjoying our life and pleasant daily activities. We understand that an illness or injury may not only affect us physically, but emotionally and financially as well. Our Medical Social Workers are prepared to bring experience in counseling through these current situations.
Our Social Workers can help with financial counseling, grief counseling, emotional support, community resources, and community involvement. Victims of tragedies are not always the only ones affected; their families may also need help coping with the aftermath of such incidences. Forever Young Home Health Care Agency’s Social Workers care about every individual we service and they will provide the very best relief possible in every situation.
Through our Social Work Program, we help patients and their families deal with the problems associated with these unpleasant experiences. We are able to help patients with advance directives as well as other long term care issues. Our Medical Social Workers are dedicated to making sure that our patients emotional and social needs are met and maintained throughout their treatment. Our programs will help to slowly bridge the gap between the incident and the individual and allow them to live life to the fullest again.
